Doesn't look bad... a little too far inside IMO on the takeaway but I've seen a lot worse.

Try taking away from the ball straighter back the first few inches or so.  Here are a couple of good videos:

I agree. Backswing doesn't look to bad. However if you are going to have a deeper (more in and behind you) backswing... on the downswing you have to bring the hands and club back out on the plane while you rotate hard left. Looks like your hands are dropping straight down to start the downswing and you are getting stuck with your club path too far in to out.

Why are you riseing up through impact? Take your normal address position, then move away from the ball 2-3 inches. In your back swing lay the club off at the top as much as you can, then swing over the top on the way down as much as you can keep in head still, while driving hips  
and rotating towards target as much as you can.Post that.

Stop both your and Hogan's swing at the same point in the backswing. Note the difference in hand path on the way down from there. Add secondary tilt till you can move your hands on the same plane and the ball goes straight , It will initially go right untill you match the tilt with your hand path.You may need to increase the space between  you and the ball a little if you start to shank it.

Nice practice swing. That's another way to do it. What is your contact like, if your natural swing is flat and you move to upright , sometimes not enough strength to pull down into the ball, thin shots result. If you hit it ok like this , you've found your swing.

These comments are worth exactly what you paid for them!  Your swing reminds me of mine.  I have fought an insider takeaway forever.  Mine has been caused by rolling my wrists clockwise early in the backswing and it appears you do too.  However, like my swing, you get into a great position at the top with a square clubface.  Many players with an inside takeaway tend to cross the line at the top with the shaft and clubface pointing to the right of the target.  You do not.  The only other issue I see is a lack of hip rotation at impact.  

Again, I am not an instructor, never played one on TV, and did not sleep at a Holiday Inn Express last night.  I've always avoided these "Here's my swing, what should I do?'" threads because there are so many people so much more qualified to chip in.  It's just that your swing reminds me so much of mine.  My long time instructor has told me not to worry about the inside takeaway because I'm in a very good position at the top, and work on my impact position.
