Left arm stuck to my chest and flipping

Extreme inconsistency.  I'm not really sure where to go from here.  I've been working on some of Monte's stuff, but it's been harder than it looks.  Any advice on a couple of things to start with?

Arm swing and shoulder turn too flat.

Right knee firing way too early.

Get a little more left tilt toward ball at the top.  Left shoulder down.
Right knee needs to feel like it doesn’t move in transition.  Increase width between knees to start downswing.
Did you see him smacked up
And cracked up
With his tongue on his chin
And his club in his hand,
Swinging from the rafters
Like a real RocknRolla
