I think the last is the form that's used in the construction of the shoe that give it its basic shape. Think of the form as a substitute for a foot in the shoe manufacturing process.
The welt is the part of the shoe that is probably what you're referring to.
Footjoy Classics aren't made with a leather sole anymore, but they still use a welted construction. If you're willing to put up with a small blemish, you can find great deals on blems on the bay or at Golf Shoes Only.com. I have a pair of Classic Tours and they are fabulously comfortable after a few round of breaking them in.
I talked to a shoe store once about converting a very comfortable pair of older nice dress shoes for golf, and I was told it wasn't practical. Golf shoes are constructed a little differently so that the spike supports that are in the sole are insulated from the footbed.
I'll agree though that the Ecco World Class welted sole golf shoes are considered by many to be the best of the best.
